Issue of Death Certificate

For the issuance of a Cypriot death certificate the following documents must be submitted:
  1. Original Death Certificate issued from the country that the death occurred. The death Certificate must be Apostilled. In the case where the country is not part of the Apostille Convention (eg Zambia), the Death Certificate must be Certified by the Ministry of Foreign Affairs of the Country and the diplomatic mission of Cyprus in that country;
  2. M138 form completed;
  3. Copy of the deceased person's Cypriot ID card or passport;
  4. Consular Fee: Euro 3.42


For the issuance of the certificate kindly make an appointment, by calling us on 0123463329 from 13:00 to 15:00 from Monday to Friday except on public holidays.
