Application for the Issuance of a Clear Criminal Record Certificate

In order to apply for a Clear Criminal Record Certificate, the following documents should be presented to the Embassy/Consulate:
- A completed “Application for the Issuance of a Clear Criminal Record Certificate”, signed by the applicant in the bottom right section of the form.
- The applicant must provide a recent photograph of his/herself equal to the number of Certificates he/she requires as well as an extra photograph for filing purposes.
- A fee of the equivalent amount of €20 in local currency for each certificate that is required. A receipt of payment (F.18) will be issued and copy of this must accompany the application.
- A copy of the applicant’s ID (for Cypriot nationals).
- A copy of the applicant’s Passport, “Temporary residence Permit” (pink slip) and “Alien Book” (for foreign nationals).
The Certificate is issued by the police authorities of the Republic of Cyprus and has a validity of two months.
